  • To contact the emergency contact
    person as indicated by the client or other health care provider on your medical
    form in an emergency situation.

Once information isreviewed, it is discarded or stored in our files. If we make material changes in the collection of the personally identifiable information we will inform you by placing a notice on the Website.
